Phase-II CAB Designation to Industry Canada
Required Documents
Required Documents (as of 5/2/2012)
The following list of documents must be submitted to NIST for U.S. CAB designation to Industry Canada, in accordance with the requirements of CB-02:
- A cover letter including:
- Request for CAB nomination to Industry Canada under Phase-II of the APEC Tel MRA
- Primary contact person's name, mailing address, phone number including extension (if any), fax, email
- Alternate contact person's name, mailing address, phone number including extension (if any), fax, email
- Desired Industry Canada Scope for Phase-II (reference: CB-02, Section 5.2)
- A copy of the Scope and Certificate of Accreditation for ISO/IEC Guide 65 specifying IC requirements (reference: CB-02, Section 5.2)
- A copy of the Scope and Certificate of Accreditation for ISO/IEC 17025 specifying the RSS and/or BETS Standards required per http://www.ic.gc.ca/eic/site/smt-gst.nsf/eng/sf09888.html
- A completed copy of the CB-02 Checklist (Annex I) – including detailed (to the section/page level) references as requested.
- A copy of all documents referenced in the CB-02 Checklist.
- Provide the names of personnel for 1) testing function (for in-house laboratory), 2) evaluation function, and 3) decision on certification function (person(s) who signs the certificate and takes full responsibility for it).
